Driving seems to be one of the last strongholds of freedom when we get older. You were so excited when you first got your license and that taste of independence. It’s reasonable to expect that you will be reluctant to give it up under any circumstance. That is what makes it so hard when your aging parent needs to hang up their fuzzy dice and you have to be the one to tell them.

We all have complained about older people on the road. They drive too slowly or they drive big cars where they can barely see over the steering wheel. We fear they may roll right over us because they can’t see us or their reactions are slow. This is the natural result of aging and it is a problem that a pharmacy cannot give you a drug for.

There may come a time when you have to address the fact that your parent is getting too old or too ill to drive their car. What do you do? Here are a few tips to help you recognize the situation and present the idea to your parents without offending them:

How Well Do Your Parents Drive?

The best way to do this is to go for a ride in their car with them. This is not drivers education; you are not teaching them how to drive better. Instead, you are looking for signs of diminished skills and reflexes, like missing stop signs, or not following traffic signals. If they seem nervous or jittery while driving, this is a sign of a problem.

Do Your Parents Have Trouble With Their Vision?

Some older people have vision problems but are loathe to admit it and avoid seeing the eye doctor. Schedule an appointment for your elder parent. Reassure your parent that they may not need anything more than a new pair of glasses and that this is not like getting an mri or a spinal tap. Seeing the eye doctor does not automatically mean they will lose their license to drive. It may just clear up a vision problem and they’ll be back on the road before they know it.

Sit Down With Your Parents

Ask your parent about driving and if they feel that they can still manage. You may get a suspicious look or an argumentative tone, but don’t join in. Remember that you are talking about their independence. Think how you would feel if someone tried to take away your license. Don’t treat your parents like children; just find out how they feel about driving. You might be surprised to find out they have been waiting for someone to ask.

Consult Your Family Physician

Your parent’s doctor won’t likely discuss your parent’s medical chart with you, that’s confidential. However, you may want to enlist the aid of the family doctor to discuss driving with your parent during their next appointment. Your parent may not want to hear it from you, but they will listen to their trusted family physician. If your parent is denying they have vision problems or judgment issues and continue to drive, it may result in an accident. For this reason, a visit with the family physician voicing your concerns would be expected. Your parent’s doctor or physical therapy specialist may have to be the one to tell Mom or Dad that their driving needs to be limited or eliminated altogether.

It is not easy to lose your independence at any age. When safety issues are at stake, you may have to deal with the fact that your parents are no longer able to drive by themselves. Enlist all the help and support you can get if and when this decision needs to be made. If you approach this issue from a perspective of concern for the health and safety of your parents and the public, then you are more likely to see positive results.

Hearing Aids : The Basics Explained

The technology behind hearing aids is changing and evolving constantly and there are many different types of hearing aid on the market today. The traditional style of hearing aid – and the one you will most often see – is the behind-the-ear aid. This type of aid has a molded ear piece which sits inside the ear and connects through a small tube to the plastic case which contains the amplification system and battery. This type of hearing aid is mainly used for mild hearing loss.

Although behind-the-ear hearing aids have given many years of service to users all over the world, comfort certainly isn’t their strong point. An alternative option is an in-the-ear hearing aid. This type of hearing aid is molded to the user’s ear and sits inside the outer ear bowl, or concha. Although in-the-ear hearing aids are designed to treat problems ranging from mild to severe hearing loss, those at the more severe end of the scale may experience significant feedback when using this type of hearing aid.

While the behind-the-ear style of hearing aid may suit many users, the sound quality can sometimes be poor. The receiver in the canal (RIC) hearing aid, however, solves many of these problems. Rather than the speaker being placed in the plastic casing and the sound traveling through a plastic tube, the speaker is placed directly in the molded ear piece, reducing distortion and sound quality problems. In addition, the plastic box and tubing are far less noticeable, giving cosmetic benefits.

Most hearing aids are temporary, while hearing problems tend to be permanent, To counteract this, a new style of hearing aid – the extended wear hearing aid – has been created to give a semi-permanent solution to the wearer. Extended wear hearing aids are designed to be worn for between one and three months and are placed inside the deep ear canal. This type of hearing aid can be uncomfortable, though, and there are many restrictions regarding their usage in conjunction with water.

If you suffer from conductive or unilateral hearing loss, your hearing professional may fit a bone anchored hearing aid. This is anchored to the skull and processes vibrations before sending them direct to the inner ear, bypassing the outer and middle ears. For medical insurance purposes, the bone anchored hearing aid is not a hearing aid at all – it is covered as a sound processor known as a Baha – and is used in cases of extreme or specialized hearing loss.

This will give you the opportunity to tell the doctor about any concerns you may have. Finally, before you go, stock up on food and toiletries. If your loved one can’t leave his home without assistance, these things can easily run out before anyone else notices.

It is also crucial that the caregiver take some time for himself during Holiday visits. Try to get enough sleep and eat well-balanced meals. This will allow you to be as productive as possible. real estate investing A caregiver must be able to determine a senior’s capacity for some activities. A seniors self-confidence is lost when they feel frustrated that they can’t do something right or can’t remember the rules of a game. Therefore, it is important to remember that an activity itself is the goal, not a specific outcome.

Simple repetition can be very good for cognition as well. Many tasks that might otherwise seem boring and uninteresting are perfect as an activity for Alzheimer’s because they can contribute a sense of accomplishment. Caregivers should enable the senior to continue in activities that utilize skills they still retain from before the onset of the disease.

According to many research studies, some mild physical activity is most recommended for those suffering cognitive dysfunction because of the increase of oxygen to the brain. This can aide in slowing down the progress of the disease, and it helps elevate a senior’s mood and promotes relaxation. Inactivity can contribute to a senior’s susceptibility to depression because they feel unproductive. They may begin to feel they can’t do something only because they aren’t encouraged to do it. This is where a caregiver must exercise an intuitive sense of what the senior is actually capable of, and should be doing to keep their body and mind occupied.

Simple and repetitive activities, such as folding towels or laundry, sweeping, polishing, mixing, and walks, can help reduce anxiety. The repetitive nature of these activities eliminates the need for memory and judgement and can give the senior a sense of usefulness.

It is important to keep an activity appropriate to their ability. To over-stimulate a senior with memory difficulty can add frustration for both the senior and the caregiver. It may also cause the senior to withdraw further to avoid embarrassment and frustration.

Here is a list of some activities for Alzheimer’s to consider:

  • Simple games
  • puzzles
  • watering plants
  • dusting
  • polishing
  • flower arranging
  • playing cards

What is Geriatric Care Management?

Introduction

First of all, let’s deal with the name itself, “Geriatric Care Management”.

To tell the truth, we don’t really like the term “Geriatric Care Management”.  After all, what client wants to be thought of as:

  • Geriatric?
  • Needing assistance?
  • Needing to be managed?

Nonetheless, the term is an industry standard and so for now we’ll stick with it. In the future, someone may think of a better name for this professional field. For now, we’ll forget about the name and talk about what it IS.

So, What Is Geriatric Care Management?

Geriatric Care Management (often referred to by its acronym, “GCM”) is a set of services performed by trained professionals, usually social workers or nurses.  Those services include, but are not limited to, the following:

  1. Assess the care needs of older adults and their families by conducting a geriatrics social work assessment.
  2. Create care plans for the acquisition of services to take care of those needs, subject to agreement by family members or trusted advisors such as elder law attorneys or trustees.
  3. Implement the care plans.
  4. Advocate for clients when needed, which often occurs when other service providers or are unresponsive,
  5. Monitor the results and make changes as needed, and,
  6. Report as required to family, referral sources, and other authorized persons.

This series of steps all sounds very tidy and orderly. Actually, it almost never happens that way in real life.

In fact, most GCM cases begin with some acute problem or crisis which requires immediate fixing and action.

After the acute problem is solved, in most cases the family chooses to have the GCM continue to be involved to monitor the client’s status and needs on an ongoing basis. Sometimes, the client’s family or friends resume control of the day to day care needs of the client and the GCM is not involved after that unless a new problem arises.

Most GCM clients are older adults who live within the GCM’s service area and whose family or friends are either:

  • Not living nearby and therefore are not able to help as much as they would like to, or
  • Working full time and therefore not able to help as much as they would like to.

The family or friends ask the GCM to begin providing GCM services to solve the acute problem, if there is one, and then to monitor and provide “family assurance visits and reports” on a periodic basis if they so desire.

Advocacy

Note number 4 in the list above, “Advocate for the client when needed, which often occurs when other service providers are unresponsive”.  Although this may not sound important, it can sometimes be critically important when considering the needs of elderly persons.

Many seniors are too “polite” to be assertive and insist upon the service and attention to which they are entitled. This often occurs in medical settings where the staff is overwhelmed with large numbers of patients and demands for their time.

It can happen in other contexts as well, such as when dealing with insurance companies or governmental agencies, where the staff attends to the needs of the “squeaky wheel”. When this happens, it results in the senior being marginalized and the senior’s needs being neglected.

An experienced professional GCM is able to recognize such situations. The GCM can take charge of the situation and make sure that the client’s needs and concerns are taken care of in an appropriate and timely manner. That is what “advocating for the client” means.

Should You Wait Until A Crisis Before Contacting A Geriatric Care Manager?

No!  Many times, that is what families do, but, No!

When a situation reaches the crisis stage, it may be impossible to completely undo the damage that has been done.  For example, it may be difficult or impossible to overcome or recover from for the effects of:

  • Falls
  • Inability to call for help when needed
  • Changes in condition that are not detected promptly
  • Theft by dishonest friends, family members or caregivers
  • Accidents arising from driving when the person is no longer safe to drive

Even if the physical effects of some problems can be reversed, the emotional and financial consequences are often never completely overcome.

Therefore it is best to anticipate problems and avoid them or plan for them before they occur.  This is exactly what we mean by “assessing care needs” and “creating a care plan”.

How is Geriatric Care Management Paid For?

Most GCMs charge for their services on an hourly fee basis. Care management services are not covered under health insurance or Medicare, so they are usually paid for without reimbursement by clients and their families.  In some cases, long-term care insurance policies may cover part of the cost of GCM services, but that is not always the case.

Summary

Professional GCM services can be a valuable source of help for both crisis resolution and for ensuring that the long-term care needs of older adults and their families are taken care of in a thoughtful and thorough manner.  Professional care managers who are experienced and educated in the needs of older adults can help families anticipate and avoid breakdowns and minimize costs of care through needs assessment, planning and monitoring.

Dementia, which is a gradual loss of certain brain functions due to the onset of mental illness such as Alzheimer s, is a fairly common disorder in the elderly. Patients face various problems like memory loss, loss of knowledge of time, and disorientation, and they have to be looked after by professionally qualified individuals.

There are two ways of caring for dementia patients   recruiting nurses at home who have special training in caring for these patients, or admitting them in a home for the aged that has special provisions for attending to people suffering from the disease.

Caring for a dementia patient, whether it is a parent or a relative, can be very agonizing for the family members, as the patient would start forgetting them and would keep losing contact with reality. Moreover, it is not possible for the family to give full care to the patient at home as they will have other commitments such as jobs and children. Considering this, it will be a wiser decision to place the patient in the capable hands of nurses and doctors at a dementia care home.

Nevertheless, you should be cautious when taking such an important decision in order to provide the most suitable care and treatment for the dementia patient – Visit dementia care homes for more details.

It is necessary to take a personal tour of the dementia care home in order to examine it thoroughly and see what facilities it has for patients. The care-takers should have the professional skills required for looking after dementia patients and they should make sure that the patients don’t forget their daily routine like sleeping on time or taking proper food, and that they do not lose their way. A considerable degree of patience is required from the care takers of dementia sufferers.

A good dementia care home should also have provisions for patients to engage themselves in constructive and creative activities and retain some social interaction skills in order to ease the symptoms and reduce the debilitative effects.

Tub chairs can significantly improve the lifestyle of the physically challenged and elderly. Using these chairs reduce the risk of slipping while taking a shower or being unable to lift oneself out of the bath tub. Nurses and caregivers can use these chairs to bathe patients.

The bottoms of all tub chairs have properties that prevent slipping. At least one well-built handle is provided with almost all tub chairs. A person suffering from physical weakness may need additional features to be completely safe while bathing.

For enhanced safety, slip-reducing appliques should be used at the base of the tub. Handrails in and just outside the tub enhance bath safety even further. Be sure to look at dementia residential homes for additional tips on caring safely for elderly or infirm people.

Many tub chairs have sliding seats that enable a patient to sit on the seat while it is outside of the bath. Either the patient or the facilitator can slide the seat inside the bath. The risk of a slip in the bath tub is reduced by this. If you’re seeking these features there are many companies in the US that produce tub chairs, that the ship worldwide.

Some types of tub chairs are customized for very heavy individuals. The special chairs make it easier for such people to take a good bath. It is a well-known fact that a mop bath, which is seen, as an alternative to a normal bath, does not remove bacteria from the skin folds effectively. Accumulated bacteria can, among other problems, cause infections.

Certain types of tub seats are designed for the more severe cases where the user needs someone’s help to sit up. It is possible to tilt these seats within an angle of forty to 90 degrees. A lot of them use belts to secure the occupant. These chairs provide a safe bathing option to patients, who otherwise would have no choice but to be sponged in their bed.
